IMPORTANT INFORMATION - PLEASE READ BEFORE COMPLETING APPLICATION

To protect the public health, there are four presidential proclamations that suspend entry into the United States of all noncitizens who are physically present in any of the below 33 countries during the 14-day period preceding their entry or attempted entry into the United States.  They are Presidential Proclamation 9984 (China); Presidential Proclamation 9992 (Iran);  Presidential Proclamation 10143 (Schengen Area, United Kingdom, Ireland, Brazil, and South Africa); and Presidential Proclamation 10199 (India).

Non-US citizen players and player support coming from or passing through these countries in the 14-days prior to entering the US  will therefore require a national interest exemption in order to enter the United States:

• China, Iran, India
• Austria, Belgium, Czech Republic, Denmark, Estonia, Finland, France, Germany, Greece, Hungary, Iceland, Italy, Latvia, Liechtenstein, Lithuania, Luxembourg, Malta, Netherlands, Norway, Poland, Portugal, Slovakia, Slovenia, Spain, Sweden, Switzerland, Monaco, San Marino, Vatican City

This country list is subject to change by the US Department of State. If you are traveling from a country not named above, e.g. Australia, New Zealand, Japan, Canada etc. you DO NOT need to fill out this form.

This application does not guarantee that you will be granted a national interest exemption, nor can we confirm a timeline for approval, which is at the discretion of the US Government. Filling out this application does not grant acceptance into the J1 College Park. Only those players accepted into the tournament and Top 25 alternates will qualify for a national interest exemption.

Incomplete or late applications regarding a player and/or guest will not be processed and waivers will not be granted. All boxes on the form are required to be completed. Only submit ONE application per player and per guest. Guest must be trainer, coach/ physio, or parent (only if the player is a minor). Spouses and other guest roles are not permitted.

Departure airport code is mandatory to include.

IF YOU RECEIVED A ONE-YEAR WAIVER AFTER SEPTEMBER 1st, YOU DO NOT NEED TO FILL OUT THIS APPLICATION (please check the status of your support person.)

United States Citizens and Legal Permanent Residents do not require a waiver, even if you have been to a country as part of the ban.

Notice regarding Data Collection for 2021 International Open of Southern California
This notice sets forth the information that the USTA will be collecting about you, how the information may be used, to whom the information may be disclosed and the disposition of the information following the conclusion of the events.  

What Is Being Collected?
In order to be granted an 212(f) athlete waiver status by the US Department of Homeland Security (DHS), to allow entry into the United States (pursuant to the DHS notice  National Interest Exemption From Presidential Proclamations 9984, 9992, 9993, And 9996 Regarding Novel Coronavirus For Certain Professional Athletes And Their Essential Staff And Dependents) USTA will collect and provide the following information to the United States Olympic and Paralympic Committee (USOPC) who will facilitate entry: Name, country of origin, passport number, date of birth, email address,  cell phone number and address while in the United States.

While participating in the Pro Circuit, you may be subject to COVID-19 testing. We may also collect your name, date of birth, email address, cell phone number and address while in the United States to facilitate COVID-19 testing by designated third party contractors.  If you receive a positive COVID-19 test result, the USTA may need to collect from you additional information for contact tracing purposes, which includes any information to reveal your whereabouts, the identity of individuals you have or may have been in close contact with and possibly exposed or infected with COVID-19, which is in addition to the information that will be requested by the local authorized health agency conducting contact tracing for the State.

How Will Information Be Used?
Your information is being collected by the USTA and will be used only for ensuring entry into the United States and compliance with the COVID-19 testing Protocols which includes safety protocols and risk mitigation measures, contact tracing and to help ensure the health and safety of everyone who will be at our premises.  Your personal information will be maintained as confidential and only disclosed as necessary to the USPOC, the US Department of Homeland Security, USTA medical personnel, the tournament directors’ offices, designated contact tracing entities if requested by them, the International Tennis Federation, and to any federal, state or local health authorities as required by law.  

How Long Will Information be Maintained?
All information in the possession of the USTA as described above will be deleted or destroyed by December 31, 2021 (or 90 days after December 31, 2021, whichever is later), unless prohibited by law.
