School Registration Appointment Request Form
School Registration for the 2022-2023 school year are done by appointments only, Monday- Thursday 7:30am-9:30am and will start July 7, 2022. Use this request form to fill out some basic information to prepare for our appointment. At the end of this form, you will select 3 preferred options for your appointment date and time. You will receive a phone and email regarding your requested date and time. If confirmed, you will receive a phone call and an email a week prior to the date chosen and a reminder phone call and email the day before your appointment. To expedite the registration process, please have documents faxed from previous school(s), our fax number is 757-892-3210. Thank you and we look forward to seeing you all!

Required documents for registration (hard copies in hand day of appointment, if not provided we will reschedule)

*Completed registration packet, packet can be picked up at Lake Taylor High School Main Office

*Address Verification- Lease/purchase (mortgage) agreement, signed within the last calendar year and a utility bill (Dominion Virginia Power, Virginia Natural Gas, or water bill) that was issued in the last 30 days

*Record birth date and number from State Birth Certificate or Affidavit

*Picture I.D. of Parent(s)/Legal Guardian(s)

*Custody orders (if applicable)

*Withdrawal papers, report card, and transcript from previous school

*Required Immunization:
4- DPT with 1 on or after 4th birthday
1- Tdap shot (cannot be more than 5 years before entering 6th grade)
4- OPV or EIPV with 1 on or after 4th birthday,
 MMR (Measles/Mumps/Rubella) all grades, 2 with the first one administered after 365 days of age
3- HBV,
 Varicella (all children born on or after January 1, 1997)

*Medication Administration or Skilled Care Procedure (during school hours)

*Special Education Services (if applicable)- 504 and/or IEP

*ESL (if applicable)

* Out of District approval letter (if applicable)
