3+2Consent and Notification of Services for KMS students (6-8)

Guardians/Parents of KMS Students, 

KRS 158.191, passed by the Kentucky General Assembly on 3/29/2023, requires parent/guardian consent and notification to address issues not academic related. Consent is required now to see our school counselors, mental health providers, and school nurses.

Throughout the school year, the school mental health providers may have small groups or individual counseling sessions with students that include but are not limited to topics on anxiety, depression, peer conflict, behavior, and emotion management.  *There may be situations when an individual student reach out to a school mental health counselor or health professional on topics that involve human sexuality, contraception, or family planning. In these situations, we will notify a parent/ guardian regardless of the consent. 

Once provided, consent is ongoing for the balance of this school year.  Consent may be revoked and shall only be effective when written notice of the revocation is received by the counseling office at the respective physical location of your student.

If you have more than one child in our district, please complete a separate form for each child.
