Please complete the form below if you need to pick up any ESSENTIAL items (Chromebook, Medication, Paper Assignment Packet) from school. You must complete this form in order to collect any item from MHS. You WILL NOT be allowed to enter the building for any reason and you must remain in your vehicle. Your items will be retrieved from your locker by school staff and delivered to your vehicle. Pick up will be 10AM-12PM Monday, April 6 or Thursday, April 9 in the car rider line of MHS. Paper assignment packets will be available on April 9. Per Nurse Willingham, any medication not picked up will be disposed of due to state regulations.
