COVID-19: Declaration of Health and Operational Compliance

All members and/or guests must complete and submit this form (the “Declaration”) to the North York Winter Tennis Club (the "Club") before being permitted access to the Club grounds or premises.
The Club requires your cooperation to enable it to open for play during the pandemic. As players, we are accustomed to observing the etiquette and rules of tennis. With the COVID-19 global pandemic, we all must commit to strictly observing some additional rules and procedures, given the personal health implications we all face from this extremely contagious virus.
The resumption and continuation of play at the Club will rely upon the assurances that all members make in this Declaration. This is essential for the safety of the Club, our members, and our staff. Accordingly, any false statements made in this declaration or any failure on your part to honour your commitment to comply strictly with our rules for COVID-19 tennis will result in you being required to leave the Club's property immediately. Additionally, you may be subject to further disciplinary measures and, in some circumstances, you may be exposed to legal liability.
As always, each member must fully abide by all rules and regulations prescribed by the Club from time to time.
The COVID-19 Coronavirus disease (the "Virus") may be transmitted by asymptomatic people and, accordingly, no measures taken by the Club and none of the statements required in this declaration can provide certainty that the Virus will not be transmitted.
Although the Club is taking prudent steps to impose and enforce appropriate protocols to keep our members safe, there can be no assurance that the Virus will not be contracted by members and guests who attend on our premises. That is a risk that you must assess for yourself and your family: if you choose to play, you are assuming the risk of contracting the Virus and of then transmitting it to others.
By completing and submitting this Declaration, you acknowledge and confirm that you are voluntarily assuming all risks of contracting the Virus and of then transmitting it to others.

Guidelines: Before Arrival at Community Tennis Club. All those who enter the club should self-screen to limit the introduction of infection:Prior to attending the Club, all those who enter the Club must do the self-assessment for COVID-19 on the Ontario Ministry of Health website and if they do not pass the assessment, they should not attend the Club for fourteen (14) days after they first pass the assessment and do not have signs and symptoms of COVID-19. Members can visit the City's website to determine if further care is required and learn about assessment centers.All those who enter the club should be aware of signs and symptoms of COVID-19, including:• Fever• Cough• Difficulty breathing• Muscle aches• Tiredness• Sore throat• Headache• Runny nose• New loss of taste or smell Your risk of severe disease may be higher if you have a weakened immune system. This may be the case for:• Seniors• People with chronic disease such as diabetes, cancer, heart, renal or chronic lung disease. I hereby declare that neither I, nor anyone in my household has contracted the Virus and is currently at risk of spreading the Virus. If I or anyone in my household has contracted the Virus or contracts the Virus after submitting this form, I will notify the Club immediately in writing by email to membership@nywintertennisclub.com and will not visit the Club for a minimum period of 14 days after my doctor has advised me that neither I nor any of the members of my household are any longer at risk of spreading the Virus, and I have notified the Club accordingly in writing by email to membership@nywintertennisclub.com. My obligation in this regard will remain in effect until the Board of Directors of the Club has declared that these requirements are no longer in effect.
I hereby declare that neither I, nor anyone in my household is experiencing, or in the last 14 days has experienced any symptoms of the Virus including any cold or flu-like symptoms, fever, cough, sore throat, respiratory illness or difficulty breathing. If I or anyone in my household experiences any such symptoms after submitting this form, I will notify the Club immediately in writing by email to membership@nywintertennisclub.com and will then not visit the Club for a minimum period of 14 days after those symptoms have completely disappeared and I have notified the Club accordingly in writing by email to membership@nywintertennisclub.com. My obligation in this regard will remain in effect until the Board of Directors of the Club has declared that these requirements are no longer in effect.
I hereby declare that neither I, nor anyone in my household has traveled to, or had a lay-over in, or has been in transit through any country outside Canada at any time in the past 14 days. If I or anyone in my household travels to, has a lay-over in, or transits through any country outside Canada after submitting this form, I will notify the Club immediately in writing by email to membership@nywintertennisclub.com and will then not visit the Club for a minimum period of 14 days after the date of return to Canada. My obligation in this regard will remain in effect until the Board of Directors of the Club has declared that these requirements are no longer in effect.
I hereby declare that I have read and understand the Club's COVID-19 policies, as posted on the Club's website (the " North York Winter Tennis Club COVID-19 Protocols"); that I will continue to review them to be aware of any changes made to them from time to time; and that I will strictly comply with each and every one of the North York Winter Tennis Club COVID-19 Protocols. I understand and confirm that the Club is relying upon the statements made in this Declaration and upon my strict compliance with the North York Winter Tennis Club COVID-19 Protocols in permitting me access to the Club's facilities and that the Club has the right to withdraw such access immediately at any time if it believes, in its absolute discretion that my Declaration is untrue in any respect or that I am not in strict compliance with the North York Winter Tennis Club COVID-19 Protocols.
