ALL families will need to provide a state birth certificate and current immunizations to the school office for student's record if this has not already been done.  Students entering 11th grade, have additional immunizations and will need to provide these immunizations prior to the 1st day of the 2021-2022 school year.

CATHOLIC families, please provide the student's Baptismal record or letter from your pastor if applying as a Catholic student.
