Youth exchange - You-th in the Wild - Lithuania Application Form
This is an application form for Erasmus+ Youth Exchange "You-th in the Wild", 9 - 17 August 2020, Lithuania

CORONA VIRUS notice: Lithuania is an Amber country, meaning that applies the following:
- The government has set out the details to enable people who have been fully vaccinated with an NHS administered vaccine, plus 14 days, to travel to amber list countries without having to quarantine on their return to England, from Monday 19 July. The recommendation for people not to travel to amber list countries will also be removed from 19 July.
- The changes will come into force from Monday 19 July at 4am. Those who have been fully vaccinated with an NHS administered vaccine in the UK and are returning from amber countries will still be required to complete a pre-departure test before arrival into England, alongside a PCR test on or before day 2 after arrival. They will not have to take a day 8 test or self-isolate.
- The cheapest cost for the tests at the moment are £23.99 https://www.find-travel-test-provider.service.gov.uk/test-type/amber
- You must take the test in the 3 days before the service on which you will arrive in England departs. For example, if you travel directly to England on Friday, you can take the test on or after Tuesday and will need to have the negative result available before boarding on Friday. Committment:
1. I am aware that I have to make a bank fee transfer of £20 immediately after the acceptance

2. I am aware that I have to purchase traveling tickets, which will be paid back 100% after the youth exchange when all the traveling documents are presented.

3. Covid testing will be compensated from the traveling budget of €275 per person. If the traveling + covid testing costs exceeds €275, it will be covered by participant.

4. I have read all the information on Abroadship.org, got familiar with all details and am fully ready to participate in the project

5. In case of selection I commit to participate in the full duration of the project.

6. I agree that I am responsible for my own health insurance (European Health Insurance Card is FREE), and that by providing the above information on special needs does not remove my responsibility for my own health and safety.
