This form is for you to nominate as an official to assist the Hockey Albury Wodonga Representative teams as a Coach, Manager or Umpire.

We currently need Coaches, Managers and Umpires for the Hockey ACT June Long Weekend Carnival.

By submitting this form you agree to abide by the Hockey Australia Code of Conduct and are also bound by the Hockey Albury Wodonga Representative Code of Conduct which is on the HAW website.  
You are also committing to have a Working with Children check if needed.

Selection for the Representative team for the June Long weekend carnival will occur in April, date TBA.

Your assistance will be required.

The Hockey ACT June Long Weekend Carnival is held on the weekend of Saturday 11th to Monday 13th June 2022 in Canberra.

U11 at Southwell Park Grass Fields, Lyneham
U13 at National Hockey Centre, Lyneham & Tuggeranong Hockey Park, Greenway
U15 at National Hockey Centre, Lyneham & Tuggeranong Hockey Park, Greenway

This form must be completed and submitted to HAW before 31st March 2022.
