We are deeply concerned about the Safety & Security of our Students & Staff. For this reason, we're asking you to complete this form.

Please complete this form, to the best of your knowledge, each time your child will be late, or needs to be picked up early from Al-Falah Academy.

Parents must come to Door One for this process and sign in or sign out their child. Again, no parents or visitors are allowed inside the building. ***No Late Arrivals after 11:05am.*** ***No Early Dismissals after 12:30pm.***
