If you are in need of a list of the sessions you attended, and you feel that the certificate(s) you received does not reflect your attendance, please complete this form.  While there are no guarantees, we will check the records to see if corrections can be made.  However, please note, if you did not initially sign-in (including not completing the google doc once you scanned the QR code, if attending in-person), we may not be able to go back to add you.  Also, it was required that you attend the entire session, with the exception of sessions that had technical issues.  

Once submitted, please give us time to review and respond.  As always, thanks for your patience!

A few other points to note:

* There was no Preliminary General Session on Saturday, so there was no CEU awarded.
* Due to the unforeseen cancellation of the approved keynote speaker for the General Session on Sunday, there was no CEU awarded for that session.
* There was no CEU awarded for the luncheon on Monday.
* Only Part One of "Where Do We Belong - A Film About Determination" was approved for CEU credit.  So, there was no CEU awarded for Part Two.
* As it was a late addition, there was no CEU awarded for "Hand In and Hand Off for Post-secondary Education Homeless Students Best Practice Roundtable."
* You had to attend the sessions in real-time to be awarded CEUs.  Although the recordings are available, sessions were approved for CEU credit only when either attending in-person or watching the live-stream.
