Please fill out a new form for EACH child that will be participating.  

All information will remain confidential to Vacation Bible Camp staff.

On the first day, please feel free to come anytime after 8:40am for the initial registration process.
In addition to registration on the first day, the parent/guardian/authorized individual(s) will need to sign the child(ren) in and out each day of camp.

Tuesday-Friday, please join us no earlier than 8:50am for Check-in each morning of camp.

This will apply throughout my child's tenure at Shantz Mennonite Church Vacation Bible School.

SPECIAL NOTE: Children in pre-school are invited to participate with their parents/guardians in one or both of our daily worship times to enjoy music, drama, and offering. This will occur from 9:10-9:40 am and 11:30 -12:00 am each day.
