Before and After school Waitlist

Hello. 

You are requesting to be placed on the waitlist for Before and or After Care. A completed registration form and the registration fee are required in order to be added to a waiting list. 


2022-23 Before/Afterschool Fees & Payment Policies

Fees are:  $35 registration fee per child (Non refundable)

 A completed registration form and the registration fee are required in order to be added to a waiting list. 

The length of the waiting list and its movement varies from grade groupings.  The Discovery Lounge cannot guarantee or predict when space will become available. Openings are offered as space becomes available throughout the year. 

Waiting list families have the following options when offered a space in the program:  Accept the space and enroll in the program by paying the weekly fee immediately. If enrolled, must attend the program. Tuition may not be paid to ‘hold’ a space. Or Decline. 

DECLINING YOUR SPACE - You can decline the space and be removed from the waiting list. (If the family wishes to enroll in the future, they are considered ‘new to The Discovery Lounge’ and must submit a new registration form and another registration fee.) or you can Decline the offered space and move to the bottom of the current waiting list (one time only).  If contacted a second time, either enroll in the program or decline and be removed from the waiting list. 

WAITING LIST CAPACITY – All registration forms will be accepted during the open registration period. At any time after the open registration process, it is possible for a waiting list to be closed because it has exceeded the number that can reasonably be accommodated throughout the remainder of the school year. Once we get enough students within a group, we will increase staff to accommodate. We must have a least 10 on the waitlist in that group to open the class. 

WAITING LIST PRIORITY 
Families remaining on the waiting who have another child in the program or has been offered a space in a grade group where space has opened. 
QCSS/TMSA Staff members take top priority and will be offered a space before anyone on the list. B

Terms, Permissions, Waivers and Conditions
You agree that the operator may authorize the physician of his/her choice to provide emergency care in the event that neither you nor the family physician can be contacted immediately.

You understand that the facility will notify you each time your child is to participate in an activity that would involve transportation other than daily travel to and/or from school.

You have read and agree with all policies and procedures set by The Discovery Lounge, Inc. When your child is absent, you understand that your normal weekly rate is required. NO EXCEPTIONS. Weekly fees are not prorated. You agree to the two week notice period when removing your child

You agree that THE DISCOVERY LOUNGE,  defines what is acceptable behavior and can dismiss a child from the program based on both parental and student behavior

You agree and understand THE DISCOVERY LOUNGE,  has the right modify their prices at any time during the year at their discretion.

These Terms are between THE DISCOVERY LOUNGE,  Inc. and its affiliates, and “you,” the individual completing this form.   You agree that this form is like any written negotiated agreement signed by you.
