2022-2023 ASES Program Request Form (Waitlist)
All information on this form will be confidential.  

This is NOT a registration form.  

Registration for the 2022-2023 school year has closed.  In the event that spots in the program open, we will begin calling families on the waitlist.  If you are interested in being put on the waitlist for the ASES program, please fill out the following form in its entirety.  (Please note this waitlist is for BOTH the AM and PM programs.  It is not for ASES sports.)   

First priority enrollment into ASES will be given to students who are identified as foster or homeless youth, as defined by the federal McKinney-Vento Homeless Assistance Act.  Second priority will be given to students that can attend 5 days a week for the full duration of the program.  Aside from these stipulations, we will go through the waitlist in the order of submission.

This request form does NOT constitute acceptance into the ASES program. This is NOT a registration form.
