By signing this form, I hereby consent to the collection and processing by Virginia Food, Inc. (“VFI”) and its respective affiliates, related parties, and agents of my:

(all as defined under the Data Privacy Act of 2012 and its implementing rules and regulations)

as disclosed by myself in this Request for Quotation Form. I hereby confirm that all information given by me herein are true and correct. I hereby authorize VFI and its respective affiliates, related parties, and agents to verify and investigate such disclosed information from whatever sources that VFI and its respective affiliates, related parties, and agents may consider appropriate.

Specifically, I hereby authorize VFI and its respective affiliates, related parties, and agents:

a. To process my order/s as specified in this form and any other document that I executed in relation thereof;
b. To notify and update me of VFI products and provide promotional advertisements, loyalty and reward offers, exclusive invites, discounts, surveys, and other marketing offers that VFI and its affiliates, related parties, and agents may deem relevant and beneficial to their customers;
c. To comply with the legal and regulatory requirements of the government; and
d. For any other purpose/s in connection with my transaction with VFI.
