INFORMATION COLLECTED ON THIS FORM WILL BE KEPT CONFIDENTIAL.

To support our MPS food-insecure students, we are providing “To-Go” meal packages that include a week's worth of lunches and breakfasts (5 lunches/5 breakfasts).  Our food services department is preparing these meals and packaging them to be picked up every MONDAY between 11:00 AM - 1:00 PM.  Parents/Guardians are to drive to the back of the Quashnet School (Door #8) and remain in their vehicles.  A staff member will bring the packaged meals for the week out to each family’s car.  If your family needs this service and is unable to go to the Quashnet School, please contact your school principal.  If you have already completed this form, you do not need to do it again--just note the change to a once-a-week pick-up.
