I understand the next step in my application is to pick up the Teacher Evaluation forms (in Guidance) and to give to a teacher for each subject I am interested in tutoring. 

For example, if you check Spanish and science courses, you would need two recommendation forms. You give one to a teach who taught your Spanish and one to a teacher who taught your science. This can either be a current teacher OR past teacher.
