This form authorizes us to send your John Jay transcript, recommendation letters, and school profile to be sent to the colleges you have communicated to us through this form.

You are responsible for sending your application and standardized test scores.  You send your SAT and ACT  scores directly from either the College Board website or ACT website.  That is the only way that they are truly official and gives you the ability to send the scores of your choice, so the colleges can superscore your results.  The applications and scores almost always come with a fee, so please make sure that you complete any necessary payments.

When and if it's required; outside transcripts from any other institution where you have received college credit must be requested by you directly from that college.  If you need to send AP scores, you must do so through College Board.  Typically you do not need to do this until after you have committed to one college, then you send it to that college.

This form MUST be completed AT LEAST 2 weeks prior to your earliest deadline.  ALL submissions MUST be submitted by DECEMBER 1st.

IF YOUR COLLEGE REQUIRES MID-YEAR OR OTHER GRADES, DO NOT USE THIS FORM FOR THAT REQUEST.  PLEASE USE THE SPECIFIC FIRST QUARTER/MID YEAR REQUEST FORM.
