Personalize lessons in Google forms

Several Google tools could be used to present content and guide a learner through learning activities. What I find missing from Google docs is the opportunity to collect response data from multiple students. Each student or a small group of students could be provided a copy of a doc with "space" for learner responses, comments, etc., but this number of copies necessary for individualizing a class makes this approach messy. Forms are not as versatile as docs or sites, but does provide a way to collect input from multiple users (stored in a spreadsheet) making tasks and mastery checks easy to follow.

This form is intended as a demo - it makes use of sections as a way to present content, images and video to convey information, and questions to collect information. It would be nice if the section description would allow the inclusion of media elements other than text and this limits the visual appeal of what can be accomplished with forms, but for those seeking an inexpensive and easy approach what I describe here is workable.
