Leadership Awards Nomination
Nominate someone for a Leadership Award for the 2021 FHDA Student Leadership Conference in Winter 2021! You can fill this out as many times as you would like and nominate multiple individuals and organizations for the same award. All awards will be chosen by the selection committee and be presented at the Conference. We will send a thank you to every nominee for their service and let them know they have been nominated.

NOTE: We use student IDs to ensure that all nominations come from FHDA students, please either use your .edu email or your student ID to verify that you are part of the FHDA district. Any nominations without this will be disqualified.

There will be one award per campus, so you can choose either campus to nominate. The categories are:
Game Changer Award - This award recognizes an extraordinary student leader from Foothill or De Anza who has redefined their position, role, or responsibility.

Extraordinary Mentor - This award helps recognize mentors who help to nurture, inspire, and grow us. You can nominate any Foothill or De Anza student who has helped you grow during your time in community college.

Emerging Leader Award - This is for a first-year Foothill or De Anza student who has stood out to you for their exceptional leadership skills, hard work, or student-oriented service.

Social Change Award - This award is to recognize a Foothill or De Anza student who has worked to make a positive difference in our local, state, national, or global communities.

Outstanding Program Award - This award is for the Foothill or De Anza Program which has served its community, gone above and beyond to help their students, or undertaken a great project during the last year. You can nominate a program or a club for this award.

Outstanding Event Award - This award is to recognize an individual or organization at Foothill or De Anza that held an event that helped the community.

Faculty & Staff Shoutout - This is to recognize faculty members at Foothill or De Anza who has significantly helped you or aided in your growth. We will be doing shoutouts for multiple faculty throughout the conference and might reach out to you to film, your own shout out. Head to https://forms.gle/89oEmStBq53GH7RU8 to submit a shoutout!
