Throughout the school year we would like to use students' photographs to highlight accomplishments. Examples of places where students' photos may be used are: in the hallways, for slideshow presentations, in the yearbook, in the local newspaper, on the website, school Facebook page, school Twitter page, or other areas if applicable.

If we do not receive a completed form, we will assume that you are in agreement with having your child's picture utilized for school-related purposes.
