Please keep your registration confirmation email, so that you will be able to edit your registration by clicking "Edit Your Response", in case there is any change in your registration. Then resubmit the form.
When you submit this Eddy Farm Volunteer Registration Form(hereafter referred to as the Form), you agree to volunteer to perform the following work without any compensation during the volunteering period:
- Building or room cleaning or other housekeeping work
- Kitchen & dinning room service
- Landscaping maintenance
- Transportation
- All other work assigned by the Eddy Farm management
When you submit the Form, you agree, during the volunteering period, to be responsible for your own health care. Eddy Farm or CCHC will not provide any health insurance or health care compensation or subsidies.
When you submit the Form, you agree, during the volunteering period, the Eddy Farm management or CCHC will not withhold any tax for you.
When you submit the Form, you agree, during the volunteering period, the Eddy Farm management or CCHC will not provide any workers' compensation insurance.
When you submit the Form, you agree that the volunteer agreement shall not be in effect until after the Eddy Farm management notifies you in writing.
When you submit the Form, you agree that the volunteer agreement shall not exceed a maximum period of three months. The volunteer must reregister the Form to become a volunteer upon the written notification of the Eddy Farm management
When you submit the Form, you understand that you can terminate your volunteer status anytime and in which case you must leave the premises of Eddy Farm within 3 days.
When you submit the Form, you agree that the Eddy Farm management has the right to terminate this volunteer agreement and revoke your volunteer status. You must leave the premises of Eddy Farm with your luggage within 7 days.
When you submit the Form, you agree that with absence of three days without written permission from the Eddy Farm management will lead to the termination of your volunteer status and your departure of the premises of Eddy Farm.
When you submit this Form, you agree that upon your departure of the premises of Eddy Farm, the room you were assigned to stay will automatically be returned to the Eddy Farm management and the management has the right to dispose of all the materials and goods in the room.
When you submit this Form, you understand that the Eddy Farm management will provide free meals and lodging, and you further understand that such provision does not constitute any compensation for the volunteer work you have performed.
When you submit this Form, you understand that the Eddy Farm management will reimburse you for the transportation cost (including fuel and toll). In order to be reimbursed for such cost the volunteer must provide all the receipts, and such reimbursement shall not exceed a maximum amount of $25.00/day.

當您提交此登記表，您同意無需任何報酬、義務于協議期間在角聲生命莊園做（但不限於） 以下工作：
- 大樓或客房清潔衛生整理
- 厨房餐廳服務
- 戶外園林維護
- 交通運輸
- 莊園佈置分配的其他任何工作
當您提交此登記表，您同意于協議期間負責自己的醫療照顧 ，莊園將不提供任何醫療保險或費用補助；
當您提交此登記表，您同意于協議期間，莊園將不為您繳納或預扣任何收入稅；
當您提交此登記表，您同意于協議期間，莊園將不提供任何勞工保險；
當您提交此登記表，您同意須經莊園的書面同意，協工協議方才正式開始生效；
當您提交此登記表，您同意此種協工安排將不超過三個月。如需延期，則須另外重新登記成爲莊園協工，并且必須經莊園書面同意；
當您提交此登記表，您明白您可以隨時停止向莊園提供義務服務，停止義務工作三天内必須帶同行李離開莊園，並通知莊園；
當您提交此登記表，您同意莊園可以隨時停止您在莊園的義務工作，停止工作后七天内必須離開莊園；
當您提交此登記表，您同意未經書面許可離開莊園三天以上將視作自動停止義務工作並視作自動離開莊園；
當您提交此登記表，您同意離開莊園后，您所住的房間將自動退還給莊園，房間内的物品將由莊園自由處理；
當您提交此登記表，您明白莊園將在您工作期間免費向您提供食宿；您明白此種食宿的免費提供并不是對您的工作的補償;
當您提交此登記表，您明白莊園將對您工作期間產生的交通費用（包括汽油費、路橋費）予以補償，協工須提交交通費用收據，並且此種補償每個工作日不得超過$25/曰。
