To help the nutrition services team plan and prepare enough 5-Day Meal Kits for each site, please complete this form if you would like to participate weekly in the FREE 5-Day Meal Kit Program for your child(ren) 18 and under for curbside pick-up. We need you to take this new survey to better serve you, now that we have added a 5th site.

1). Lake Stevens High School, service 10 am to Noon
2). Glenwood Elementary, 10 am to Noon
3). Lake Stevens Middle School, 4 pm to 6 pm
4). North Lake Middle School, 4 pm to 6 pm

We will do our very best to anticipate the trends and counts at each location weekly. This survey response does not guarantee your child(ren) meal kits, the data is only being used as a forecasting and planning tool.

Please note that due to a USDA waiver extension all Meal Kits will continue to be FREE through the end of the 2020-21 school year. Each Meal Kit will contain 5 day's worth of school breakfasts and lunches. Milk will be optional at pick-up.
