In submitting this form I understand and acknowledge the terms and conditions of The DASH Academy LLC, co-directed by Lindsey Pettus  and Gregory Dolbashian. In consideration of the benefits of instructions provided by DASH Academy LLC and its instructors, I, intending to be legally bound, participating in the event and do hereby waive claim and release DASH Academy LLC, Directors, Faculty, and/or any other instructors, and/or other personnel for claim or liability for any injury, or accident, occurring or arising from the instructional program or incidental sponsored activities either on or off these premises. I further acknowledge that DASH Academy LLC can not guarantee that I will not become infected with the Coronavirus/Covid-19. I voluntarily seek services provided by DASH Academy LLC and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures including wearing a mask and social distancing requirements to reduce the spread while attending the event hosted by DASH Academy LLC. By choosing to participate in DASH Academy LLC events, as indicated below and submitting this form, I authorize DASH Academy LLC to use my image in part or whole in publications and/or on the website, thedashensemble.org and/or any social media associated with DASH Academy LLC or The DASH Ensemble.
