ANY STUDENT NOT COMPLETING THIS FORM WILL REMAIN OR WILL BECOME A TRADITIONAL LEARNER ONCE THE SIGN-UP WINDOW CLOSES ON SEPTEMBER 18, 2020.
PCSS recognizes there are continued concerns from some of our families and students as we continue through the COVID-19 outbreak in Putnam County. This concern specifically relates to school attendance in a traditional building.
The following agreement will allow a current student to become or remain a Remote-Learner and be able to attend and complete all required school work from a remote setting, as long as strict attendance and participation expectations are continuously met. This agreement will be in effect for the remainder of the 2020-21 School Year beginning on Monday, October 19, 2020.
Students with a diagnosed medical condition preventing them from either school-based attendance or remote attendance may be eligible for instructional service through a Homebound educational plan which is not a part of the Remote-Learner policy.
Please complete one (1) form for each student you wish to participate in or remain in the REVISED remote-learner plan for the remainder of the 2020-2021 school year.
