QUK University League - Southern 2 (Oxford) - Medical Information

This form is intended for everyone participating at the University Southern Fixture at Oxford High School as a player, playing volunteer, or non-playing volunteer. This form should be completed by January 23rd.

In order to compete or volunteer at the fixture, all players and volunteers must provide

(a) any relevant medical information

(b) up-to-date emergency contact details.
