I also confer that I / they are physically and mentally capable to undertake the activities run.

I understand that I / my children may be photographed at Platinum Taekwon-Do or UWA Taekwon-Do and give consent for these photographs to be used in future UWA Taekwon-Do or Platinum Taekwon-Do promotions. To the best of my knowledge all information submitted in this form is correct.
