EARLY BIRD Registration is $125.00 per golfer ($500 per foursome) and $140 after Friday May 8th 2020. All registrations must be paid in full by Friday May 22, 2020.

Registration Includes: 18 holes of golf, cart, steak dinner, door prizes, skill prizes, hole-in-one prize, and silent auction.

Please fill in one form per team. Payment will be collected separate than this form. Your totals will not be calculated so please know your total amount when sending payment. Instructions for payment will follow the completion of this form.
