Oregon Film Hiring Data Worksheet - Individual Self-Identification
Oregon Film requires all incentivized projects to share specific hiring data with the office as part of our effort to identify and recognize the needs of all members of our community.  

We collect names and contact information as that allows us to tie out the data provided with other reports and metrics we gather as part of the project's audit. Individual data will not be shared publicly. Anonymously aggregated reporting to better identify ways in which our programs can better serve and reflect our community as a whole will be generated.

We suggest productions ask employees to self-identify at the time of hiring by including this worksheet as part of your Start Form packets. Worksheets can then be sent individually or collated into a single worksheet and submitted as part of the audit process performed at the completion of production work in Oregon.

We appreciate your compliance as the more comprehensive the data, the more informed our work will be going forward.
