1. Please complete and submit this form to express interest in our summer camp programs.  Please note submission of this form is not guarantee of registration.
2.  Offical Camp registration begins on April 7, 2024 at 11 am at Advent Lutheran Church.  
3.  Payment in Full is expected at the time of registration to secure your spot.
4.  Payment can be accepted by one of the following methods: Cash, Check, or Credit Card.
5. If you have more than 1 child interested in camp, please complete this form for each child separately.

Campers will need a packed lunch, water bottle, and sunscreen (all labeled please). Please send a swimsuit and beach towel on Thursday.  Healthy, fun snacks are provided. A pizza lunch is provided on Friday.

All registered camp weeks must be paid in advance to secure their spot. All applicants must complete an Enrollment Form to request child's spot in the program, which is due at the time of registration. In addition, each child must complete a Child Information Record with emergency contact information, and each family must acknowledge the camp handbook and camp policies. All forms must be on file with the Camp PRIOR to your child attending the program.   *
I understand that Summer Adventure Camp captures photos and videos of campers throughout the program. These photos may be used in camp communications and marketing, including print and online. No child is ever identified by name by Summer Adventure Camp without written permission of the parent/guardian. *
