Parents, please complete this form by Friday, 27 November 2020 to cancel your son's individual instrumental or vocal tuition, to take effect from the beginning of 2021. Please be aware that according to the Music Tuition Terms and Conditions, parents must complete this form at least 4 weeks before the end of term for the cancellation to take effect at the end of the term.
