PARTICIPATION WAIVER FORM
Please read the following carefully. Students MUST agree to the following waiver conditions prior to any participation in any activity planned and/or organized by our church and/or our youth group. If you are under 18 years of age, your parent or guardian MUST be the one to complete this form. As such, please go over this waiver with your legal parent and/or guardian if you are under 18 years of age.
SECTION I - PERMISSION: I hereby grant my child (myself if over 18) full permission to participate in any and all Thrive Youth activities planned and organized by The Drive Baptist Church (“The Drive”) and/or The Drive’s youth group staff and sponsors. *
SECTION II - WAIVER AND LIABILITY: I hereby release The Drive, along with its staff and sponsors from any and all responsibility and liability for any injury, illness (including COVID-19 - see Section V), or death that I/my child if under 18 may incur in his or her participation in any youth group activities. *
SECTION III - MEDICAL CONSENT: In the event of a medical emergency, I hereby consent for myself/my child (if under 18) to receive any necessary medical care as directed by a physician if he/she is in need of such care AND I am unable to be contacted. *
SECTION IV - MEDIA RELEASE: I hereby grant permission for myself/my child (if under 18) to be photographed and/or filmed by The Drive and its staff and sponsors for the sole purpose of social and promotional material related to activities associated with Thrive Youth and/or The Drive. Furthermore, I hereby release The Drive, its officers, directors, volunteers, and staff from any and all responsibility and liability that may arise as a result of such use of photographs, videos, and/or other media. *
SECTION V - COVID-19: Should in-person gatherings occur, I hereby agree that I/my child (if under 18) will adhere to all health and safety guidelines as required by The Drive Baptist Church, which follow the recommendations and requirements of New Brunswick Public Health and the New Brunswick Government. *
By answering "yes" below, I declare that a) I am 18 years of age or older, OR I am the parent or legal guardian of the student indicated on this form who is under 18 years of age AND b) I understand and agree to any and all conditions, regulations, permissions, etc. that are associated with participating in Thrive Youth/The Drive Baptist Church events. *
