I acknowledge that there are inherent risks associated with participating in the sport of volleyball.

By Signing this form, I agree to not hold Twin Bridges Volleyball Club or its coaches responsible for any injury or damages caused by my participation in activities associated with this Club.

I will allow Twin Bridges to take pictures of Myself/My Child for the purpose of promoting the sport of volleyball, through their website and social media.

Furthermore, with respect to the Twin Bridges Volleyball Club, I acknowledge that any activities I participate in, are of my own free will and volition.

I will notify the coach if Myself/My Child will not be at practice and the reason. (personal or sick)

I understand that Twin Bridges Volleyball Club must follow all local and provincial COVID-19 guidelines. If there are changes in mandates around vaccination, screening and /or masking, I understand my child will be required to follow these guidelines or they will be unable to participate in any Club events.
