Waiver & Media Release: My child will be a participant in the "Izzy Ingle Basketball Camp" at Timpanogos High School conducted by the coaches and players of Timpanogos High School. I hereby waive all claims for injury or accident of liability of any kind against Timpanogos High School and any or all employees, coaches or student coaches participating in this camp/clinic. I am aware that injuries will be covered by my own family insurance. If an emergency does happen, I am aware the coaches will make all attempts to contact a guardian using the information provided above. And if they are not reached, I give permission for my athlete to be seen by the athletic trainer or other coaches. I also understand the camp may need to photograph or film students participating in the basketball drills, games, etc. as coverage of the positive event. By checking the "yes" box on this permission form, I am indicating that I am agreeing to allow the use of such materials for the upcoming school year (2020-2021). *
