I, [parent/guardian name], hereby give my consent for my child, [child's name], to participate in youth football and/or cheerleading programs organized by Washington Park Buccaneers. I understand the risks involved in such activities and acknowledge that my child's participation is voluntary.

I agree to comply with all the rules and regulations set forth by the organization, including those related to safety measures, equipment, conduct, and participation. I understand that failure to comply with these rules and regulations may result in my child's exclusion from participation in the programs.

I acknowledge that participation in youth football and cheerleading activities involves certain risks, including but not limited to, bodily injury, paralysis, disability, or death. I understand that the risks may result from the inherent nature of the activities, the acts or omissions of other participants, or the condition of the facilities where the activities take place.

In consideration of my child's participation in the youth football and/or cheerleading programs, I, on behalf of myself and my child, hereby release and discharge Washington Park Buccaneers, its officers, directors, employees, volunteers, agents, and sponsors from any and all liability for any and all injuries, losses, damages, or expenses arising out of or related to my child's participation in the programs, whether caused by negligence, breach of contract, or otherwise.

I agree to indemnify, defend, and hold harmless Washington Park Buccaneers, its officers, directors, employees, volunteers, agents, and sponsors from any and all claims, demands, actions, or causes of action arising out of or related to my child's participation in the programs, including but not limited to, any claims brought by third parties, such as other participants or spectators.

I acknowledge that I have read this consent and liability waiver carefully and fully understand its contents. I am aware that I am giving up substantial rights, including my right to sue. I sign this waiver voluntarily and without any inducement, assurance, or guarantee being made to me, and intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
