2022-2023 OFMS Chromebook Opt-Out Form
Please complete all of the following questions in regards to technology usage and Chromebooks with the Orchard Farm School District.  

Please submit one form for each child that you have at Orchard Farm Middle School that would like to opt-out of the OFSD Chromebook Program.

Please submit this form no later than Friday, August 19, 2022.

Agreement to Care for the Personal Device:  (1) I will take good care of my home device so that it is in working order for school. (2) I will never leave my home device unattended in an unsecured or unsupervised location.  (3) I will know where my home device is at all times.  (4) I will charge my home device battery to full capacity each night and bring it to school daily. (5) I will protect my home device by always carrying it in a secure manner to avoid damage.  (6) I will use my home device in ways that are appropriate for education while at school.  (7) I have read and will follow the policies outlined in the District Acceptable Use Policy while at school as well as outside the school day.  (8) I will file a police report in case of theft.  (9) I will be responsible for all damage or loss/theft caused by neglect or abuse. *
Technology Acceptable Use Agreement:  According to the Orchard Farm R-V School Board document EHB-R: “The district’s technology resources may be used by authorized students, employees, School Board members and other persons such as consultants, legal counsel and independent contractors.  All users must agree to follow the district’s policies and procedures.  Unless authorized by the superintendent or designee, all users must have a signed User Agreement on file with the district before they are allowed access to district technology resources.”  (The entire Technology Acceptable Use policy can be accessed electronically on our District School Board Policy website:https://simbli.eboardsolutions.com/ePolicy/policy.aspx?PC=EHB&Sch=416&S=416&C=E&RevNo=1.11&T=A&Z=P&St=ADOPTED&PG=6&SN=true).                                            We have read and understand the 2022-23 District Technology Acceptable Use Policy.  I understand that my use of the district’s technology resources is not private and that the school district may monitor my electronic communications and all other use of district technology resources as well as downloaded material and all data I store on the district’s technology resources, including deleted files, pursuant to state and federal law, even if the district’s technology resources are accessed remotely.  I understand that this form will be effective for the duration of my attendance in the district unless revoked or changed by the district or me. *

In the event that your child would need to borrow a Chromebook for the day from the Orchard Farm School District (online testing, personal Chromebook broken or forgotten at home, etc), please read and understand the following:  I do hereby understand that I am waiving my right to purchase insurance for this device.  By waiving this right, I will be 100% responsible for any costs assigned to damage or loss/theft that is incurred while under my care, even on school property, including the cost of the case if lost/stolen. *
Care of an OFSD Borrowed Chromebook:  If I should have to borrow an OFSD Chromebook for the day, I will care for said Chromebook with the following guidelines:  (1) I will take good care of the Chromebook.  (2) I will never leave the Chromebook unattended in an unsecured or unsupervised location.  (3) I will never loan out the Chromebook to other individuals.  (4) I will know where the Chromebook is at all times.  (5) I will keep food and beverages away from the Chromebook since they may cause damage to the device.  (6) I will not disassemble any part of the Chromebook or attempt any repairs.  (7)  I will protect the Chromebook by always carrying it in a secure manner to avoid damage.  (8) I will use the Chromebook in ways that are appropriate for education.  (9) I will not place decorations (stickers, markers, writing, etc.) on the Chromebook.  (10) I understand that the Chromebook I am issued is subject to inspection at any time without notice and remains the property of Orchard Farm School District.  (11) I have read and will follow the policies outlined in the Chromebook Policy Handbook and the District Acceptable Use Policy while at school as well as outside the school day.  (12) I will file a police report in case of theft or damage caused by fire.  (13) I will be responsible for all damage or loss/theft caused by neglect or abuse.  (14) I agree to pay the full replacement cost of my Chromebook, power cord/charger, bag & ID tag in the event that any of these items are lost, stolen or intentionally damaged.  (15) I agree to return the Chromebook and case in good working condition at the end of each day.  (16) I understand that, without insurance, I am not able to take this chromebook home at any time. *
