Instow Critical Front-line Worker Booking Form for a Child's Place in School - 5th January 2021
Good Morning - In order for us to plan in this very tight turn around, please return this form by 3.00 p.m. today, Tuesday evening 5th January.
The government has asked parents to keep their children at home, wherever possible, and asked schools to remain open only for those children who absolutely need to attend because parents are front-line critical workers and their job means that they can not work from home.
Any children remaining at home will be able to access remote learning set by their teachers.
You must complete this form in order for your child to attend Instow School during this lockdown ( 5th January to February half term, or until there are other changes from the government).
Please complete one form for each child.
Who is eligible:
* Pre-school children can still attend the pre-school in the Parish Hall as normal from Tuesday to Friday mornings. Liaise with Mrs Sharon Goodwin. You do not need to complete this form.
* Instow primary aged children, whose parents are designated critical front-line workers.
* Instow children with an Education Health and Care Plan (EHCP) - you do not need to complete this form. You will be contacted in a separate and confidential email.
* Instow children who are currently vulnerable pupils and WITHOUT a pre-existing medical condition - you do not need to complete this form. You will be contacted in a separate and confidential email.
Please be aware that if our critical front-line worker numbers are too many, then we will have to organise a part-time timetable of attendance, due to site restrictions.
It is not compulsory for critical front-line worker children to attend school, it is offered on a voluntary basis until further notice.
Thank you for taking the time to use this booking form, it is vital to help us plan ahead and keep children and staff safe on site. Completing this form complies with GDPR regulations.
