SISC SMART Summer Program - Enrollment Form
Summer Enrollment Guidelines

1. Fill-out the Google Form with your complete basic information and choose the subject/s that you would want to enroll.
2. The accounting staff will send the billing and the payment instructions through email. (*Fees will not be collected if previously enrolled for SMART Summer Program Part 1)
3. Pay and email a scanned copy of the deposit slip (for bank deposit) and transaction receipt (for bank wire transfer) at accounting@southville.edu.ph
4. Once payment has been confirmed, an official Registration form will be emailed to you by the Office of the Registrar.
