2021-2022 DMS Car Tag Request Form
This school year, we will continue our 20-21 afternoon car dismissal procedures to ensure responsible distancing throughout our campus. If your scholar will be a car rider in the afternoons, please complete this form to register your scholar(s) to receive a DMS Car Tag. You will receive two laminated car tags to be displayed from your vehicle's rearview mirror. Scholars will be dismissed at car dismissal by car tag number; therefore, scholars will need to know their car tag number.
