Rising Sun's Program - Mentee Form
Rising Suns Mentor Program's purpose is to mentor Black boys, youth, and young men from the African, Caribbean, and Black communities. Priority is given to boys and youth from single-parent homes, vulnerable, marginalized, and at-risk youth. This form is the first initial step in the mentoring process. No information on this form will be shared with any outside organizations. The Rising Suns Program is anticipated to begin in January 2021. This mentor program will focus on boys and youth ages 8 to 25 years old. Parents, guardians, and community leaders who are seeking a mentor for boys, youth, and young men up to 25 years old, complete this form. Young men between the ages of 16 to 25 interested in being mentored, please complete this form too. The form is for Mentees. Mentors do not complete this form, if you are interested in being a mentor, complete the mentor form. This form is only for mentees. Parent/guardian consent for boys 16 and under.  Program Details: Boys and youth will be required to be available for a minimum of 2 to 4 hours a month. Benefits of Mentoring
What will my son benefit from the Rising Sun's Mentor Program? Dear parents and guardians, the big question is what will your son's, nephews and young men benefit from the Rising Sun's Program. Our program is culturally appropriate and focuses on Black excellence, heritage and history. We focus on the "whole development of each mentee". Our mentors are professional Black men from our communities, who serve in different careers. Our program focuses on empowering our young boys and young men, as mentees meet with their mentors on a monthly basis. Connecting Black youth to Black male role models in the community is critical to the development of our boys and young men. Through the program your son's, nephews will enhance their communication skills, life-skills, engage in personal development, network in peer groups, connect with professional Black men in the community. Your son's and nephews will also learn more about the contributions Black people have made to Canada and globally. The program focuses on school-based preparatory experiences, career and entrepreneurship development, leaderships development, and connecting activities. We ensure that all programs and services are also delivered by Black, African and Caribbean professionals from our communities. All programs and services are designed based on your son's individual strengths and needs, as we focus on Black excellence.

Process of mentorship
1. Complete the Mentorship Form
2. You will be contacting from someone from our team
3. Mentorship Interview with parents and guardians & mentorship overview
4. Parents and guardians complete all required forms (i.e., medical form, permission form, etc.)
5. Mentee's and Mentor's are partnered
6. Introduction meeting with mentors and mentee's with program coordinator
7. Mentoring begins based on both mentor and mentee schedule
8. Parents/guardians, please maintain communication with mentors for progress of your son's, nephew's etc.
