Recital Ticket Purchase Form and Pre-Registration 24-25

Please use this form to indicate your ticket purchase for Monona Academy of Dance's "Dancin Across the USA" recital on Saturday June 1st at 2 PM and Sunday June 2nd at 2 PM. You will need to fill out the form for each day that you wish to purchase tickets. One form for 6-1-24 and one form for 6-2-24. Tickets will be assigned by the date stamp of the form fill and upon the date stamp of payment of the ticket invoice.

Please enter the number of tickets you would like to purchase. Adults are $19.50 (18 and up) + tax, Youth $9.50 (17 and younger) + tax, Lap seats for (2 and younger) $1 + tax.

Please note the lap ticket is required to be able to provide fire officials with an exact number of individuals in the auditorium in case of an emergency.

Balcony seating is open for the Saturday performance only. If you would prefer to be in the balcony please indiciate your seat preference below. We will only use the first three rows. We are providing an option for you to pre order a floral bouquet for your dancer from Sun Prairie Flowers by Hen and Chick. All bouquets hand made in store by their staff. These order will be taken through Thursday May 16, 2024 at 1:00 PM. The bouquets are available in either red or pink with the 3 rose bouquets for $25 + tax and the 6 rose bouquets for $47 + tax. The bouquets will be available for pick up at intermission outside the theater doors at a clearly marked table. Pre Orders Only!

We are also preparing for the 2024-2025 Academic year by accepting your wrap around registration today. A yes, indicates your intention to return next year for a reduced registration fee of $40 + tax versus a registration fee of $63 + tax after May 29, 2024. Your wrap around registration will include a personal email in late June with class placement and schedule information for the 24-25 academic year. *
