Registration (INDIVIDUAL) - Seattle Field Hockey Social 2019
Use this form to register as an individual for the Seattle Field Hockey Social (SFHS) 2019.

Everyone will be allowed to play on two teams (e.g. mixed and women OR mixed and men). The tournament fee is $60/person regardless of whether people play in one or two divisions.  

After submitting registration, payment will be requested via an electronic invoice through Square and can be securely paid via credit or debit card.
