Hockey: registration, consent & medical form for Hastings Girls' High School
Dear Parent/Caregiver & Student,

Congratulations on being selected into your HGHS 1st XI hockey team for the 2021 season. Your team will be competing in the Hawkes Bay Hockey Secondary Schools competition. The competition commences in Term 2 and Division 1 and 2 starts on the 12th/13th May and will run for 13 weeks until 18th/19th August. Games will be held on Thursday afternoons. Here is some information you'll need to be aware of:

Games/matches:
Weekly games are played at secondary schools (some will be played at home and other games are away).
note: Please remember to check the game fixture on a weekly basis in case of any changes. These can be found on the Hawkes Bay Hockey website or on the sports notice board by the small gym at HGHS

Transport:
The school can provide transport to & from the games as required.
note: We will also appreciate any assistance from any parents/caregivers that can assist with transporting the students and this includes driving the students in the school vans if we're short on drivers.


Costs:
$150 per person (this cost covers registration fee, warm up tops, hoodies, transport, first aid, equipment, uniform etc.) Please note this cost does not include tournament week costs.

Please pay fees directly to the student centre at school or pay directly to the following bank account: Hastings Girls High School, 12-3145-0140200-00 (reference student name + your sport).

Here's what gear you need:
- school hockey dress (provided but must be returned to the school at the end of the season)
- warm up tops (provided)
- hockey sticks
- shin guards
- mouth guard
-  covered footwear
- water bottle
- optional: towel

The Hastings Girls' High School Code of Conduct that all students have signed, describes in detail our expectations regarding conduct. We require exemplary behaviour from the girls at all times. School rules will apply and any misdemeanors will be dealt with in a more serious manner than they would be at school. Please speak with your daughter about her conduct while on the trip and encourage her to co-operate fully.

The following will be required of your daughter/ward:
1. Pay her fees at least 2 weeks prior to the competition/tournament or have a payment plan in place. No pay, no play.
2. Have this consent form completed at least 2 weeks prior to the competition/tournament.
3. Follow the school code of conduct at all times
4. The school may use your daughter/ward's photograph or video images in school newsletters, websites, social media and other promotional material.
5. Return any issued uniforms at the end of the season in good condition. If you don't return your uniform then the cost to replace the uniform will be added to your school fees.
6. Transport games may be made by way of school van, private car or taxi as required.
7. Your daughter/ward must be at school on game days to be able to participate on that day.
8. Tournament Guidelines (should your daughter/ward be selected for a tournament team); Your daughter must have at least 85% school attendance record; Your daughter must have a satisfactory pastoral record (this includes disciplinary, school work completion and other aspects of school life); Failure to meet these guidelines may result in your daughter being excluded from the tournament team.

*If you agree to the terms at the bottom of the page you are making a commitment to attend ALL trials, trainings and games for the entire competition/season and pay fees on time.

By completing this form I am acknowledging that myself and my daughter have read and accepted the Hastings Girls' High School Sports student contract, parent contract & the code of conduct.

Student Contract
1. To attend all required meetings, training sessions, and be available for all designated games
2. I agree to always be on time and will notify my coach in advance if I am going to be late
3. I agree to inform my Coach, Manager or Teacher In Charge of any circumstance if am unable to attend
4. I agree to conform to the rules of the game, and student code of conduct at all times
5. I agree to my photo being used for publicity purposes. Write in the comments box above if you do not agree.
6. I agree to my name, phone number and address being circulated amongst Coaches, Managers and the TIC
7. I will represent Hastings Girls' High School with pride and wear the correct uniform proudly
8. I agree to follow the school code of conduct at all times
9. I agree to return any sports uniform and equipment immediately after the tournament/competition

Parent Contract
1. I give consent for my child to play sport for Hastings' Girls High School
2. I agree to support my daughter in Sport at Hastings Girls' High School by allowing my initial contact details to be shared with the Coaches, Managers and TIC
3. I understand all fees are payable prior to participation in competition. Non-refundable. This is review-able at the discretion of the TIC.
4. I agree to ensure my daughter will adhere to their above contract
5. I understand that where possible transport will be arranged for girls who are playing games immediately after school (3.10pm-5pm). I understand that girls playing later than this will need to organise their own transport and that I am required to collect/arrange collection from the event at the end of each game/match. Transport will be provided for all sporting events that take place during school hours (unless specified otherwise).
6. If a specific tournament or event is scheduled during school time, I give permission for my daughter to represent the School during that time (Information regarding such events is always given in advance with parental signoff at that time)
7. I understand and agree that during the season it may be necessary for my daughter to travel to venues outside of school, and occasionally leave school prior to 3.10pm due to time constraints.


HGHS Sport Code of Conduct
As a participant in the Hastings Girls High School Sports programme, I will always conduct myself in a manner to the following standard:
I agree to:
1. Wear the Hastings Girls' sports uniform with pride at all times and follow all school rules
2. Always play by the rules of the sport
3. Never argue with an official. If I disagree with a decision I will inform the captain, coach or manager during a break or after the competition.
4. Control my temper. I understand that verbal abuse of officials, other players or spectators or deliberately distracting or provoking an opponent is not acceptable or permitted behaviour in any sport.
5. Work equally hard for myself and my team. Play to 'win' but always play fair.
6. Demonstrate commitment to my team through being punctual and participating fully in all scheduled activities including practices, and games.
7. Show sportsmanship and applaud all good plays, whether they are made by my team or the opposition.
8. Treat all players as I like to be treated. I will not harass or take unfair advantage of another competitor.
9. Respect my coach, teammates and opponents.
10. Display modesty in victory and graciousness in defeat.
11. Respect the rights, dignity and worth of all participants regardless of their gender, ability, cultural background or religion.
12. Thank the opposition and officials at the end of the game.
13. As a spectator or reserve, I will only be positive in my comments and nor cheer on negative or dangerous behaviour.
14. Comply with anti-doping policies.
15. Understand that the sports uniform is property of the school and to be cared for the duration of the season/competition after which time is to be returned to the school.
PENALTIES: I understand that failure to abide by this Code of Conduct could exclude me from participating in Hastings Girls' High School sports programmes and teams.
