Please complete this survey to request a Chromebook for Springfield Elementary School students who do not have a Chromebook to bring to school on a daily basis or to let SES know of those who will be bringing a from home. A personal device brought from home needs to be a computer or laptop (not a tablet, iPad, cell phone or another device), and the student should be able to bring it to school on a daily basis. Please understand that the chromebook will be required to travel with your child to and from school daily.

This form is for Kindergarten - 5th Grade students only.

For those requesting a chromebook, after you complete the form and review the information, the school will distribute a Chromebook to your child at school. Information submitted in the survey should be accurate so that it can be verified by school officials and you may be reached if needed. If you have any questions, you may contact the school.

In completing this request and receiving a Chromebook for your child, you agree to maintain the Chromebook and return it to the school in the same condition in which it was received. You also agree to be charged $360 for a device if it is lost or damaged beyond repair. Furthermore, a reasonable repair cost will be assessed to cover repairs for broken keyboards, screens, or charging cables.
