I understand that all volleyball activities have inherent risks and I hereby assume all risks and hazards that can occur during my participation in all volleyball training activities including transportation to and from said activities. I further waive, release, absolve, indemnify, and agree to hold harmless Aaron Garcia, the organizers, volunteers, supervisors, officers, directors, participants, coaches, referees as well as persons or parents transporting participants to or from such activities from any claims or injury sustained during my use of any facilities utilized by Coach Aaron Garcia or participation in any volleyball activities.

PERMISSION FOR TRANSPORTATION: Aaron Garcia has my permission to transport my child to and from program location in the event of an emergency. I understand reasonable precautions will be taken to ensure the safety and supervision of my child.

Dear Parent/Guardian:

During the Camp or trainings, we take photographs and Videos of the volleyball players and athlete's by which incidentally, some photographs may capture your child's participation, directly or indirectly.

These photos may be published through our website, social media pages, news bulletins, billboards, and ads.

With this, we seek for your consent in allowing us to publish photos which may involve your child to the said platforms.
