I acknowledge that my son/daughter's participation in the HDMLA Field Development Camps involces risk to them and others.  I hereby voluntarily accept all suck risks, both known and unknown, and any and all consequesnces withich may arise from those risks.

In consideration of the HDLMA allowing me to participate in the  Field Development Camps (the receipt and sufficiency of which is herby acknowledged), I, myself, my dependents, heirs, executors, succesors, assigns and personal representatives, hereby waive any and all claims of any kind or nature which I may have against the HDLMA, its parent company, subsidiaries and affiliates and each of their respective officers, directors, and volunteers.(collectively herinafter referred to as the "Released Parties") arising directly or indirectly from my participation in the Field Development Camps  including any and all claims for personal injury, death, illness property damage, property loss or econimic loss, even where such clains result from the negligence of the HDMLA or such released parties.  I hereby undertake and agree that I will not bring any legal proceedings in any court or tribunal asserting any such claims against any of the HDLMA or such Released Parties.  I hereby agree to indemify and hold the HDMLA and such Released Parties harmless from any such claims.  

I further state and certify to the Released Parties that (a) I am over the age of 18 years or if I am under the age of 18 years, I have the consent of my parent or legal guardian to participate in the Field Development Camps ; (b) I am familiar with the activities involved in the Field Development Camps ; (c) I am aware of the safety  requirements and safety equipment neccesary to participate in the Field Development Camps ; (d)the HDLMA has advised me of the safety equipment that I will be personally responsible for providing to participate in the Field Development Camps  and (e) I am in adequate physical condition to participate in the activities invovled in the Field Development Camps .

I expressly acknowledge that participation in the Field Development Camps , by its very nature could, in some circumstances, be hazardous, dangerous and result in physical injury.  I acknowledge that certian behaviours on my part could increase that risk associated with the Field Development Camps , including my failure to use proper or adequate safety equipment and failing to excerise due care and attention.
