Please answer the questions below so we have updated contact information and general information about your child as we begin our work together.  The location of the training sessions is indoors at LIMMA- 590 Smith Street, Farmingdale, NY 11735 and training packages are available as per the information below.

Check the box to indicate the session(s) you are interested in.  The cost of the ten session package is $350 (check one box below).   All sessions will run from the week of March 1, 2022 through May 12, 2022. *

I understand that the cost of each ten-session package is $350 must be paid by February 18, 2022 to reserve your child's space.  I understand that the price must be paid upfront via cash or Venmo @Beth-McCoy-26, unless otherwise agreed upon between the client and CBM Athletics.  If a student athlete misses a session due to a conflict, they can make it up on a different day/time that week or another week.  No refunds will be given for sessions not utilized. *If I begin after the week of March 1, 2022 a prorated agreement will be made between CBM Athletics and the client. *
