Code of Conduct Incident Report Form
The Atlas of Variant Effects (AVE) Alliance is committed to creating a friendly and respectful environment and maintaining the highest level of behavioral integrity in our community. AVE  members and participants in our events, workstreams, committees and communications are expected to show respect and courtesy to others. If you have a concern or feel that the AVE Code of Conduct (https://www.varianteffect.org/code-of-conduct) was violated, please complete this form to report the incident. 

You may also make a report directly to Lara Muffley (muffley@uw.edu) or any member of the AVE Executive Committee by contacting them via email, or in-person at an AVE event.

This form is anonymous, unless you decide to provide your contact information. We can't follow up an anonymous report with you directly, but we will fully investigate it and take appropriate action as necessary to prevent a recurrence.
