Impilo Swimming  - Cancellation of Enrollment form
This form is to completed if you are cancelling your swimmers enrolment, we are very sad to see you leave and wish you all the best in your future, before continuing with this form please ensure the below Terms and Conditions have been understood and that you have managed to abide with all of them.

1. Accounts are fully paid up.
2. One calendar months notice has been given
3. You are aware that all credit balances will not be paid out to you, but will be kept in your school account for future use.
4. By submitting this form, your membership will be cancelled, should you wish to return in the future you would need to pay the annual membership fee again. (Unless you are re-joining in the same year (ie stopping for a short period) then you will not be re-charged the annual fee, once your account has been re-actived we will look for available days and times.)
5. Cancelations will only be accepted if completed on this link, and you have received an acceptance letter from the school. No notice may be given verbally, emailed or sent via WhatsApp or any Short message system.
6. By submitting this form, you accept that all "make-lessons" are forfeited. unless caught up in your notice month where possible,
