Thank you for being an Open Streets Watsonville supporter! We're so excited to co-create an amazing community event with you. This form should be submitted by all participating organizations and performers to give us the details about your booth/performance/activity/demo.

IMPORTANT:

LIABILITY INSURANCE
Certificates of Liability Insurance (COI) are required from ALL participating organizations, with the exception of musicians/performers. If securing event insurance is a problem, please contact us as we may be able to assist.

WAIVER OF LIABILITY
A Waiver of Liability TO BE SIGNED BY PARTICIPANTS is required if you will be running an activity of a physical nature (e.g. bicycling, Zumba or martial arts). Certificates must name Bike Santa Cruz County Education Fund as additional insured; address: 333 Soquel Ave., Santa Cruz CA 95062. Certificate of Liability Insurance requirements: $1,000,000 minimum limit of Liability. Certificates must be from A.M. Best’s “A” rated carriers.

Please note that sales are NOT PERMITTED at Open Streets, distribution of political materials comes with certain regulations, and we make an effort toward *low-waste/footprint events* so no-waste activities, recyclables, compostables, and environmental awareness activities are encouraged. We recommend that you keep in mind the mission of Open Streets as you design your outreach.
(PERFORMERS ONLY) - What style of music, dance, or art will you be sharing?
(PERFORMERS ONLY) - what time of day do you prefer to perform/lead? Performances can range from 11am-3:30pm. Please note that we will try to accommodate you but may not be able to.
What kind of special equipment will you be setting up besides table(s) and canopy? *
PLEASE NOTE: You must provide all equipment necessary for your booth/activity, including tables, chairs, canopies, etc.
NOTE: If you are using a canopy pop-up tent, you MUST secure the canopy legs with weights or you will not be permitted to set up your tent. *
Water jugs, sandbags, or cement buckets do the trick.
What space/equipment needs do you have for your booth/performance?
Standard booth space is 10'x10', unless otherwise approved. NOTES: 1. Presenting organizations need to supply their own supplies, e.g. table, chairs and canopy. 2. ALL CANOPIES MUST BE ANCHORED WITH HEAVY WEIGHTS DUE TO WINDY CONDITIONS. 3. Power may be available but is *not guaranteed* at this time.
