By submitting this form you are agreeing to to following statements:

If I am awarded and accept a scholarship, I agree to participate actively in the event, and to provide reports on this event in the form agreed in the conditions of the scholarship.
If I am awarded and accept a scholarship, I will be responsible for the booking and costs of travel, accommodation, incidentals, and meals during the conference and will retain receipts for reimbursement by Wikimedia Australia as agreed in the conditions of the scholarship.
If I am awarded and accept a scholarship, I will be responsible for arranging my own travel insurance.

All information contained in this application will be reviewed by the members of the Wikimedia Australia Scholarship Subcommittee, and shortlisted applications will be endorsed by the Wikimedia Australia Committee.
All personal information will be held strictly confidential by the Committee.

I as an applicant for a Wikimedia Australia scholarship relating to my participation in this event, hereby acknowledge and affirmatively consent to the following as a prerequisite to the consideration of my scholarship application ('Application'):(a) Personal data that I submit via this form as part of or in relation to my Application (collectively 'my Data') may be accessed and reviewed by members of a scholarship subcommittee made up of Wikimedia Australia committee members.

This Data may be collected, stored, used, communicated, archived, destroyed or otherwise processed by members of the Scholarship Subcommittee. My Data may be retained by Wikimedia Australia (WMAU) until I request in writing to WMAU that my Data be destroyed or for as long as required by applicable law.
